Title:An electron microscopic study of lymphatics in the dermal lesions of human leprosy.

Abstract:The dermal lymphatic vessels in lepromatous and tuberculoid leprosy lesions were studied by light- and electron-microscopy. In the lepromatous patient, lymphatic vessels were seen in both intra- and peri-granulomatous areas. The lymphatic lining cells contained lipid droplets, lysosomes, and numerous pinocytotic vesicles. Cells bearing bacilli were only occasionally seen. In the tuberculoid cases, lymphatic vessels were seen only along the edges of the granulomas and the lining cells were less prominent. Inflammatory cells, both lymphocytes and histiocytes, were found traversing the walls of lymphatic vessels in both groups of patients. The results of the study confirm the continued and increased functioning of the lymphatic drainage system in dermal leprosy lesions, and indicates that it may be a major route for the clearance of lipids from the lipid-rich bacilliferous lesions in the lepromatous patient. The lymphatic pathway appears to be a minor pathway for the dissemination of Mycobacterium leprae in comparison with the blood vascular system^ien.

Abstract:(Role of temperature, microtraumatisms by elongation or subluxation and canalar stricture.) Cold has a slight aggravating effect on leprosy which is observed in some countries as Iran, South Russia, North India and the Andean cordillera. The subluxation of the ulnar nerve has facilitating effect in the development of the neuritic damage in a limited number of patients. The elongation is important for the production of the ulnar neuritis. The canalar stricture is a major factor causing nerve damage in leprosy. It explains the segmentary localization of the principal damages of the leprous nerves in the proximal areas situated above the tunnels. Intraneural hypertension appears first and is responsible for the swelling of the nerve which gets entraped inside the tunnel. A circulus viciosus is, then, created which maintains and aggravates the intraneural hypertension.

Abstract:A brief review of the contradictory opinions of different authors as to the nature and the place of tuberculoid cases of leprosy in the evolution and pathogenesis of the disease is presented. Mention is also made of their failure to agree on what could be generally considered as the esential histological features of this condition. A case of tuberculoid leprosy which has undergone transformation into a lepromatous-type case within six years of observation is presented. This case report together with the clinical, bacteriological and histological studies of other authors seem to agree with the opinion of Manalang that tuberculoid leprosy is a stage in the evolution of leprosy, not immutable as assert by Schujman. That such transformation among adult tuberculoid cases is rare because they represent the filtered resistant ones or benign infections, is indicated. The need for continued follow-up and life-long study of contact children born of leper parents in institutions is indispensable if we are to know the evolution of leprosy. (AU).

Abstract:Rheumatic manifestations are a common feature of leprosy. The present study was performed to assess the incidence of rheumatic manifestations in leprosy. Methods: seventy cases of leprosy attending the indoor and outdoor departments were studied. Results: Out of the 70 cases studied, rheumatic manifestations were seen in 61.42% of cases: arthritis in 54.28% and soft tissue rheumatism in 17.14%. Enthesitis was seen in 2.84% of cases. The incidence of arthritis in lepra reactions was 57.57%. Conclusions: Musculoskeletal manifestations can occur at any time during the infection. Articular inflammation in leprosy usually occurs in reactive states, particularly erythema nodosum leprosum (ENL). Leprosy can present with arthritis as the first symptom.

Abstract:The question of the prospect for absolute cure, oron the other hand that of transformation to the lepromatous type, of active major tuberculoid cases, especially those that at one time or another have been found bacteriologically positive, is discussed. Out of twelve such cases observed at Cebu for several years, no less than seven have relapsed at one time or another after apparently complete recession of the disease. In one instance there had been a period of five years without trouble. It is suggested that the feature of relapse in tuberculoid leprosy is worthy of more attention than it has received. Lepromatous transformations has not as yet occurred in any of these cases. Two of them, however, have persisted for three years and more in an unstable, borderline condition, the picture dominated by repeated papular eruptions, the lesions atypical clinically and histologically. Another borderline case of decidelly different aspect seen in China is cited; it presented at the same time apparently lepromatous, tuberculoid and intermediate lesions and was looked upon as transitional, yet later recession occurred as is common in ordinary tuberculoid leprosy...AU.

Abstract:The relative recurrence risk ratio lambdaR (and particularly the sibling recurrence risk ratio, lambdaS) is often of interest to those wanting to quantify the genetic contribution towards risk of disease or to discriminate between different genetic models. However, estimating lambdaR for complex diseases for which genetic and environmental risk factors are both involved is not straightforward. Ignoring environmental factors may lead to inflated estimates of lambdaR. We present a marginal model which uses a copula function to model the association in cumulative incidence rates between pairs of relatives. This model is applicable to present-state data and allows estimation of risk of disease in a pair of relatives (and hence lambdaR), given measured environmental covariates. We apply the model to leprosy among sibling pairs from the Karonga district, Malawi. If risk factors are ignored, the apparent lambdaS in this population is over 3. Accounting for known nongenetic risk factors reduces it to just under 2. (AU).

Title:The course of open cases of tuberculoid leprosy at the Cebu leprosarium.

Abstract:The clinical features and histological findings in eight tuberculoid neural-type cases in the Philippines, that were hospitalized because bacteriologically positive and observed from two to more than six years, are recorded. In seven cases the principal lesions were of major grade, moderate or very marked; but one case, though otherwise essentially similar, was never more than of minor grade. In three, if not four, other cases, after partial subsidence of the condition, some of the lesions progressed as of the minor tuberculoid variety, giving further evidence of lack of fundamental type differentiation of the ordinarily recognized(major) tuberculoid condition from the lesser forms of neuromacular leprosy. In two cases there was unilateral involvement of a nerve trunk of an extremity on which the first lesion appeared further indicating that in this form of the disease such affection may occur by extension through the cutaneous nerves, and not necessarily by metastasis. Onset of the disease was commonplace, in three instances with anesthesia preceding the appearance of macules, which in the group as a whole were usually of the simple form at first. Development of tthe condition that led to hospitalization usually, though apparently not invariably, resulted from reaction, whether mild or severe. Several cases experienced one or more relapses by reaction after they came under observation. A striking feature of the condition, hitherto not emphasized, is a secondary eruption, primarily papular or mixed papular and macular, often abundant, that can be ascribed only to metastatic dissemination of the infecting agent by may of the blood stream...(AU).

Title:Seasonal variations in the activity of lesions of neuro-macular cases of leprosy.

Abstract:I. A study has been made in individual cases of seasonal variations seen in the tuberculoid lesions of leprosy in Calcutta. Clinical axacerbations in such lesions are most commonly seen in the monthes from february to may. The phase of increased clinical activity usually lasts fro about six months. In about 20 per cent of the cases subject to clinical exacerbations there is a tendency for the exacerbations to recur. The recurrence of increased activity is usually seen in the same season as the initial exacerbations4. In about 25per cent of the cases subject to exacerbations, bacilli are found in smears taken from the lesions during the increased clinical activity. Bacilli usually disappear from the lesions before the complete subsidence of increased clinical activity.

Abstract:O A. obteve os seguintes resultados nas suas indagações: Pesquisa de bacilos nas lesões cutâneas: lidando o A. com doentes tuberculoides internados em hospitais, onde são submetidos, com regularidade, os exems bacterioscópicos mensais, os resultados das pesquisas foram relacionados com os vários exames praticados durante o período de hospitalização. Observou, como era de esperar, que a percentagem de pesquisas positivas foi tanto elevada quanto maior o número de exames efetuados. Assim, em 73 doentes em que foram praticadas de 1 a 5 pesquisa bacterioscópicas, a percentagem de exames positivos foi de 8,2;no outro extremo, em 35 doentes com mais de 32 exames bacterioscópicos, o resultado foi positivo em 40 por cento dos casos (assinala porém, alguns casos em que se procedeu inutilmente à pesquisa dos bacilos em mais de 50 esfregaços). Quanto à percentagem geral de exames positivos foi de 18 por cento, sendo considerados conjuntamente quer os tuberculoides com lesões em face de repouso quer em face reacional, perfazendo um total de 466 doentes. O A. faz notar que a cifra obtida se baseia em várias pesquisas bacterioscópicas e que, na maioria dos casos apenas um exame foi positivo entre o muitos praticados. Essas mesmas observações foram assinaladas ao considerar os resultados da: Pesquisa dos bacilos no muco nasal: no material de 600 doentes a percentagem geral dos exames positivos foi de 8,2. No material conseguido pela curetagem nasal a positividade foi de 3,2 por cento (4 curetagens positivas e 153 negativas). Pesquisa de bacilemia: em 145 doentes foi pesquisada a bacilemia, estando presente em dois casos. Pesquisa de bacilos no material de punção de gânglio: em 6,7 por cento dos casos o exame foi positivo (10 punções positivas e 138 negativas). Pesquisa de bacilos no material de punção de baço: praticada em alguns doentes, sendo encontrado um único bacilo no esfregaço. Pesquisa de bacilos no material dos "abcessos" de nervo: em 17 casos o exame foi negativo (material cedido pelo Dr. Nelson de Sousa Campos). (AU).
